Follow the steps below to agree to the form.
1.  Enter your students 7 digit student ID number (example: 0123456) and choose "lookup".  Please reach out to your student's teacher if you need your student's ID number.
2.  Once a match is found, choose the option to view the policy by clicking the link on the word "here".
3.  Read and fill out the information to accept and sign the form.
4.  You will receive a confirmation stating that you have approved the policy.
