In order for a student to be issued a Chromebook; parents or guardians need to sign two (2) digital forms.
The forms are located on page 2, Section 1 of the Chromebook Procedures and Guideline Handbook. We can only link forms from the handbook. Please open the handbook to get to the forms.
1. Please click on the Acceptable Use Guidelines and enter your child's student ID without the letter s. Fill out the form and then click submit.
2. Please click on the ELBC User Agreement Form and enter your child's student ID without the letter s. Fill out the form and then click submit.
When both forms are electronically signed; our part-time Chromebook technician will distribute a Chromebook to your child during normal school hours.
